I already receive specific provision but I feel my condition is getting worse/has changed, do I need to be re-assessed?

If you already receive specific provision but feel that your condition is getting worse/has changed, you will need to be re-assessed and should contact AccessAbility (Exeter) or the Accessibility Service (Penryn) to book an appointment as soon as possible.
